Other Details

  1. Entry fees per team is RS.7500/- 18% GST. 
  2. Entry fees to be paid on or before 31st of May 2024.
  3. T-Shirts, Breakfast and Lunch will be provided by the organizers. 
  4. Team should have 15 players compulsory. 
  5. Minimum one Doctor should be in the playing 11.
  6. All players should be from the registered branch. And there is no chances of a player from other branch of same hospital. 
  7. Maximum age limit for the player is 50 years.
  8.  Registration fee is non refundable under any                    circumstances of the tournament.
  9. The following documents should be produced before the match by the captain's:
  • Hospital ID card
  • Recent Photograph 
  • Contact details 
  • Appointment letter signed by HR
  • Current working certificate issued by HR.
